Calculator: Dynamic International Prognostic Scoring System-Plus (DIPSS-Plus) for primary myelofibrosis (PMF) in adults and adolescents

Input

Age >65 years (1 DIPSS point)

Leukocyte count >25,000/microL (>25 x 109/L) (1 DIPSS point)

Hemoglobin <10 g/dL (<100 g/L) (2 DIPSS points)

Circulating blast cells ≥1% (1 DIPSS point)

Constitutional symptoms (1 DIPSS point)

Platelet count <100,000/microL (<100 x 109/L) (1 DIPSS-Plus point)

Anemia requiring transfusion (1 DIPSS-Plus point)

Unfavorable karyotype (1 DIPSS-Plus point)

DIPSS interpretation

DIPSS point count DIPSS risk category DIPSS-Plus points
0 Low risk 0
1 to 2 Intermediate-1 risk 1
3 to 4 Intermediate-2 risk 2
5 to 6 High risk 3

DIPSS-Plus interpretation

DIPSS-Plus score DIPSS-Plus risk group Median survival (all patients)* Median survival (patients <60 years)*
0 Low risk 15.4 years 20 years
1 Intermediate-1 risk 6.5 years 14.3 years
2 to 3 Intermediate-2 risk 2.9 years 5.3 years
4 to 6 High risk 1.3 years 1.7 years

Notes
  • Constitutional symptoms include:
    • Weight loss >10% of the baseline value in the year preceding PMF diagnosis
    • Unexplained fever or excessive sweats persisting for more than one month
  • Unfavorable karyotype includes:
    • Complex karyotype rearrangements
    • One or two abnormalities that include +8, -7/7q-, i(17q), -5/5q-, 12p-, inv(3), or 11q23
  • * Total DIPSS-Plus score was applied to a group of 793 consecutive patients with PMF seen at the Mayo Clinic, and in a separate analysis, the total DIPSS-Plus score was applied to a group of 299 patients <60 years seen at the Mayo Clinic.

Equations used

DIPSS = Age + WBC + Hb + Blasts + Constitutional symptoms
DIPSS-Plus = Plt + Anemia + Karyotype + Adjusted DIPSS

Only digits 0 to 9 and a single decimal point (".") are acceptable as numeric inputs. Attempted input of other characters into a numeric field may lead to an incorrect result.
