  • Abnormal liver function: Chronic liver disease (eg, cirrhosis) or biochemical evidence of significantly impaired liver function (eg, bilirubin >2 times ULN plus one or more liver enzymes >3 times ULN)
  • Abnormal renal function: Chronic dialysis, renal transplantation, or serum creatinine ≥2.26 mg/dL (200 micromol/L)
  • Stroke: Previous history
  • Bleeding: Bleeding disorder or previous bleeding episode requiring hospitalization or transfusion
  • Labile INRs: Unstable/high INRs or <60 percent of INR values in therapeutic range
  • INR: international normalized ratio; ULN: upper limit of normal
