These MRIs are from a patient with HELLP syndrome and a subcapsular liver hematoma. (A) The T2-weighted image shows a 9 mm hyperintense focus (arrow) that could represent a small laceration or a hepatic cyst. (B) The T1-weighted image shows an adjacent subcapsular hematoma (arrowhead) with both hyperintense and hypointense components reflecting a complex solid thrombus. The subcapsular hematoma was estimated as occupying less than 10 percent of the liver surface.
MRI: magnetic resonance imaging; HELLP: hemolysis, elevated liver enzymes, and low platelets.
