The apical view from a two-dimensional echocardiogram with color flow Doppler in a patient with hypertrophic cardiomyopathy and left ventricular outflow tract obstruction shows turbulence in the outflow tract caused by contact between the anterior mitral valve leaflet and the interventricular septum. As a consenquence of the systolic anterior motion of the mitral valve, there is abnormal coaptation of the mitral leaflets, resulting in a posteriorly directed jet of mitral regurgitation.
