Initial history for women with chronic pelvic pain
Perform thorough review of systems and obtain past medical and surgical histories, with focus on past menstrual history, prior contraceptive use, and prior obstetrics and gynecologic history
Evaluate for history suggestive of diagnosis of:
  • Endometriosis
  • Pelvic inflammatory disease
  • Gastrointestinal disease, especially irritable bowel syndrome
  • Urinary disease, especially interstitial cystitis/painful bladder syndrome
  • Musculoskeletal disease
  • Psychiatric disease, with a focus on symptoms of depression and anxiety
 Assess characteristics of the pain:
  • Associated findings
  • Provocative and palliative factor
  • Quality of pain
  • Radiation of pain
  • Setting in which pain occurs
  • Temporal: Timing when the pain occurs and events that precede or trigger pain
Obtain psychosocial history, with particular focus on: 
  • Substance use or abuse, including alcohol, marijuana, and prescription medication
  • Patient safety: Does the patient have a current or past history of physical, sexual, or emotional abuse?
Ask about prior diagnostic tests or pain treatments
Inquire about family history of relevant clinical conditions, such as chronic pain syndromes
