In the Diabetes Prevention Trial, insulin therapy did not prevent type 1 diabetes in siblings of diabetic patients with high serum islet-cell antibody concentrations and low acute insulin responses to glucose. Subjects received close observation or low-dose subcutaneous insulin (ultralente insulin administered twice daily; total daily dose 0.25 units/kg) plus annual four-day continuous insulin infusions. After median follow-up of 3.7 years, 69 of 169 subjects in the intervention group and 70 of 170 subjects (42 versus 41 percent, respectively) in the observation group were diagnosed with diabetes.
