What a discharge plan should include
When you leave the hospital, the staff should give you:
A written description of why you were in the hospital
A written list of all the medicines you are supposed to take, and instructions on how to take them and how to get them
Written instructions on what to do if your condition changes or you have warning signs
Written details about all the medical appointments you have after you leave the hospital
Written instructions on how to get the results of any tests that were done that have not yet come back by the time you leave the hospital
The contact information for a staff member at the hospital who can answer your questions
