Multifocal atrial tachycardia. Lead V1 and lead II rhythm strips show an irregularly irregular narrow QRS complex rhythm that, on first glance, looks like AF with rapid ventricular response. On closer examination, there are P waves preceding each QRS complex, and, overall, there are more than 3 different P wave morphologies, consistent with the diagnosis of multifocal atrial tachycardia.
