Protocol for cytoreductive surgery with heated intraperitoneal chemotherapy
Place two large bore intravenous catheters.
Place arterial catheter.
Place thoracic epidural catheter for postoperative analgesia.
Place esophageal temperature probe.
Use goal directed fluid therapy based on SVV: maintenance Ringer's lactate, with bolus of 250 mL colloid for SVV >10% or urine output <0.5 mL/kg/hour.
Maintain goal urine output at ≥0.5 mL/kg/hour.
Administer antiemetics prior to HIPEC.
Maintain normothermia with warming measures during CRS, cooling measures during HIPEC. Turn off warming devices 15 minutes prior to HIPEC infusion. During HIPEC, aim for core temperature <39°C. If necessary, reduce HIPEC inflow temperature.
Obtain baseline laboratory studies prior to start of HIPEC: CBC, electrolytes, ABG. Repeat every 30 minutes during HIPEC and for two hours after infusion stops.
