Once the tracheal tube introducer (ie, bougie) is in place within the trachea, a 6.0 size endotracheal tube (ETT) is railroaded over the bougie until the vocal cord line above the cuff is advanced several centimeters into the stoma. Avoid placing the ETT too deeply into a mainstem bronchus; a 6- to 8-cm insertion depth is appropriate for most adults.
