The images show a young child with polydactyly (A, B). His thumb was duplicated from the metacarpophalangeal joint, and he had duplicated proximal and distal phalanges (Wassel type 4). The radial thumb was ablated, and metacarpophalangeal joint arthroplasty was performed with correction of the Z deformity (C, D).
