Tuberculous empyema
(A) Axial view of computed tomography of the thorax shows a right peripheral crescentic collection of liquid surrounded by a circumferential calcific shell (arrow), with thickening of the adjacent extrapleural layer of fat, thickening of the cortex and sclerosis of the adjacent right ribs. Due to restriction of the right lung expansion, the right hemithorax is smaller than the left and the cardiomediastinal structures are shifted to the right.
(B) Coronal multiplanar reformation of the chest CT images demonstrates the craniocaudal extent of the pleural process.
