Older Adult: High-Risk Medication:

Beers Criteria: Doxylamine, a first-generation antihistamine, is identified in the Beers Criteria as a potentially inappropriate medication to be avoided in patients 65 years and older (independent of diagnosis or condition) due to its potent anticholinergic properties resulting in increased risk of confusion, dry mouth, constipation, and other anticholinergic effects or toxicity; use should also be avoided due to reduced clearance with advanced age and tolerance associated with use as a hypnotic (Beers Criteria [AGS 2019]).

Pharmacy Quality Alliance (PQA): Doxylamine as a single agent or as part of a combination (excludes OTC products) is identified as a high-risk medication in patients 65 years and older on the PQA’s Use of High-Risk Medications in the Elderly (HRM) performance measure, a safety measure used by the Centers for Medicare and Medicaid Services (CMS) for Medicare plans (PQA 2017).

Adverse Reactions

Cardiovascular: Palpitations, tachycardia

Central nervous system: Disorientation, dizziness, drowsiness, headache, paradoxical central nervous system stimulation, vertigo

Gastrointestinal: Anorexia, constipation, diarrhea, dry mucous membranes, epigastric pain, xerostomia

Genitourinary: Dysuria, urinary retention

Ophthalmic: Blurred vision, diplopia

Contraindications

OTC labeling: When used for self-medication, do not use in children <12 years of age, with other products containing doxylamine, or inadequate time for a full night’s sleep.

Canadian labeling: Additional contraindications (not in US labeling): Hypersensitivity to doxylamine or any component of the formulation; narrow angle glaucoma; asthmatic attack; prostatic hypertrophy; stenosing peptic ulcer, pyloroduodenal obstruction; bladder-neck obstruction; concurrent use with monoamine oxidase-inhibitors.

Warnings/Precautions

Concerns related to adverse effects:

• CNS depression: May cause CNS depression, that may impair physical or mental abilities; patients must be cautioned about performing tasks which require mental alertness (eg, operating machinery, driving).

Special populations:

• Pediatric: Do not use for insomnia in children <12 years of age. Antihistamines may cause paradoxical excitation in young children.

Other warnings/precautions:

• Self-medication (OTC use): When used for self-medication (OTC), notify health care provider prior to use if you have breathing problems (eg, chronic bronchitis, emphysema), glaucoma, difficulty in urination due to enlargement of the prostate gland, or are currently taking sedatives or other sleep-aids, tranquilizers, other antihistamines, or any other medications. Discontinue use and notify health care provider if sleeplessness persists continuously for >2 weeks.

Opioid Agonists: CNS Depressants may enhance the CNS depressant effect of Opioid Agonists. Management: Avoid concomitant use of opioid agonists and benzodiazepines or other CNS depressants when possible. These agents should only be combined if alternative treatment options are inadequate. Pregnancy Considerations

Maternal use of doxylamine in combination with pyridoxine during pregnancy has not been shown to increase the baseline risk of major malformations. Doxylamine may be used for the treatment of pregnancy-associated nausea and vomiting (ACOG 2018).

Breastfeeding Considerations

Doxylamine succinate is expected to be found in breast milk although actual concentrations are not known (Madjunkova 2013).

Adverse events (unusual excitement, irritability, sedation) may be expected in a breastfeeding infant. Infants with apnea or other respiratory conditions may be more vulnerable. In general, if a breastfeeding infant is exposed to a first generation antihistamine via breast milk, they should be monitored for irritability or drowsiness (Butler 2014).

Mechanism of Action

Doxylamine competes with histamine for H1-receptor sites on effector cells; blocks chemoreceptor trigger zone, diminishes vestibular stimulation, and depresses labyrinthine function through its central anticholinergic activity.
