Calculator: Sequential Organ Failure Assessment (Quick) qSOFA score in adults

Glasgow coma score derived points 15 (normal mental status*) (0)

<15 (not normal) (1)
Systolic blood pressure   mmHg
Respiratory rate   breaths/min

Important: Inputs must be complete to perform calculation.

qSOFA score   points

qSOFA interpretation

2 to 3 points: High risk
0 to 1 point: Not high risk

  • *A Glasgow coma scale score of 15 requires all the following: Spontaneous eye opening; accurate orientation to person, place, and time; and appropriate motor responses to commands.
  • For this calculation, Respiratory rate derived points = 1 when the respiratory rate is ≥22 breaths/min, and 0 for slower rates. Systolic blood pressure derived points = 1 when the systolic blood pressure is ≤100 mmHg, and 0 for higher blood pressures.

Equations used
qSOFA_score = Glasgow_coma_score_derived_points + Respiratory_rate_derived_points + Systolic_blood_pressure_derived_points

Equation parameters, such as Glasgow coma score derived points, have two or more discrete values that may be used in the calculation. The numbers in the parentheses, eg, (0), represent the values that will be used.

Only digits 0 to 9 and a single decimal point (".") are acceptable as numeric inputs. Attempted input of other characters into a numeric field may lead to an incorrect result.
