- Age of onset after age 50 years

- Nocturnal pain or diarrhea

- Unexplained weight loss, fever, or other systemic symptoms

- Laboratory abnormalities (eg, iron deficiency anemia, elevated ESR/CRP, elevated fecal calprotectin, fecal occult blood present)

- First-degree relative with inflammatory bowel disease or colorectal cancer
